Spinal Screenings

A Spinal Screening Program is provided annually to students. Starting this February 1st Spinal Screening will be provided to the 6th grade students only. All students in special education programs, students transferring into the school who have not previously been screened, and students to be tested related to classroom teacher requests will also be screened during this annual program. Parents and teachers may request screenings of student related to specific concerns at any time. Please feel free to contact me if you have any questions or if you would like to request screening (s) for a particular student.

Medications in the Clinic

Each school year written authorization and instructions from the parent/guardian must be placed in the student’s file in the school clinic or office before the school nurse may administer any medication to a student. Please be aware that prescription and/or over-the-counter drugs must be accompanied by a physician’s written order that specifies the dosage, frequency, and directions for administration. A new authorization form must be completed for any change in medication. Telephone permission will not be accepted.

It is the responsibility of the parent/guardian of the child to provide any medication to be given at school.

All forms regarding medications given during school hours can be found in the clinic.
